We do not perform surgery or anesthetic procedures on any patients with underlying or ongoing health problems. If your pet has epilepsy, seizures, cardiac/heart disease, liver or kidney problems, hypoglycemia, immune disorder or any other health issues, we will not perform surgery or any anesthetic procedure on your pet.

Every pet needs one of these filled out before we provide any service. Save time and fill it out at home before you arrive!

More pets are returned home when micro chipped. Shelters, SPCAS, and other rescue groups scan for Homeagain Microchips to identify lost pets.

Any pet undergoing anesthesia (spay, neuter, declaw, dental, etc) needs to have this form filled out in full.

Euthanasia can be a hard time for a family. This form is required by state law. If you are planning to have a pet euthanized you can fill it out in the privacy of your own home, and shorten your stay at the clinic.
